Three for unfriendly, like five or six for hostile.
The least aggressive side in a conflict gains the ability to declare war on the more aggressive side.
All attacks, bar ambush, increase the invisible meter, but massacre, plunder, and learn are 'less' hostile than traditional march, conquest, and raze, and increase the meter slower.

So when can war be declared actually? When both kds are at hostile and only a less aggressive side can start the war?

Is there a time limit, like X hours after hostile kicked in? Or can that be done immediately after?
No. Once can be hostile and one can be unfriendly. The unfriendly kd has the options to declare.
OK. And if both are hostile, then the less aggressive one has the option?
Does size of kingdoms matter when it comes to who gets the option to declare war? Or is it just an invisible hostility meter that counts?
trad marches are 1
learns/plunders/massacres are .5
ambushes are 0.25-.50 not sure on the exact amount but they do count and are right around the L/P/M amounts
conquests are 1
failed attacks are some unknown value 0- <1 depends on attack used, amount failed by, and is always less than 1.
Amount fades on the hour change 1st of the Month.
Aggressive stance doubles these values
2.5 points takes you to unfriendly so you can do 4 learns/massacres/plunders without getting to unfriendly.
~7-7.5 points takes you to hostile if you are in aggressive stance and have a province make a quad trad march into another kingdom they will have the button.

So the opposite kd gets the war button, even if they don't do any hostilities on us?
They can only get the button if they reach unfriendly, so they have to hit to unfriendly status before they can declare.

How much does it fade every hour? A lot?
I believe around 15% or 1 point whichever is higher. The 15% is very rough, different people have different opinions.

15% decay per hour??? That's huge. Let's say I make a trad and get 1 hostility point. Then within the next say 20 hours this point decayes to something like 0.05 point??? Not possible.
