Build a lot of watchtowers and hospitals. Make it so they can't keep up with their losses.
Hospitals don't particularly help against gnomes. WT are good of course, if they actually are A/T. Make sure to check first. Do proper SoM translations, record number of elites out so you can get their defense with CB. Chain them heavily, especially if they're low on GS.

Heavy chains with pitfalls on some, keep the others depopulated (fireballs or whatever) so they can't train anything back. You need to be in it for the long run. Their losses are high, so their endurance when kept underpop isn't good at all, use that against them.
Also, make good use of dragons. Ruby drake is nice.
Another option is to mix the fireballs with Expose Thieves. Especially on the ones that only log in a few times a day to burn all their stealth. The more stealth they gather, the more you can destroy fast with ET

Sure, hospitals are fine and should be used. They just aren't higher priority against gnomes than against other races.

Gnomes are cakewalks.
They either turtle with everything at home, in which case you take away their ability to op by using expose thieves, or they have offense, which leaves them with one or more of the following: low realistic offense and defense; really low peasant-count; really bad WPA; etremely networth-inflated provinces, that don't bite nearly as hard as you'd expect them to.

The low WPA is already obvious. Offence and defense seem pretty good. Draft looks around 60-62 on average.
That offense and defense is a lot lower than any other race's, because it's mostly composed of elites - elites that never get a reduction in their death-rate.
Massacres are good, taking land is better.
Overpop and kill their armies, so they can't send out even if they want to.

Lots of massacres? Why? Just chain them. Only do massacres if you want some easy NS target (and gnomes require many to be)
It's a situational thing.
When it feels right to massacre, you know you're about to own some ass.

Ruby Dragon, Pifalls, WarHero, and go heavy on the chaining. Keep their most hard to hit attackers firballed, control their economy. Don't let them outgrow you.

2 things to keep in mind when warring gnomes
1) DO NOT let them outgrow!! gnome unbreakables r gods
2) do not let anyone get away with suiciding. keep ur intel updated
other than that u will observe situations and see wat they do and respond correctly
as for massacring, dont do it unless its ur big prov multitapping their small gnome. single tap massacres r a waste
Hmm. Thanks for the suggestions. We started with a wave of massacres. Maybe I will suggest to rethink that move for next wave.
And we ended up going with an emeral dragon. Pitfalls on all the targets BG on some and we let out 15 attacks in 2 hours. We have 9 War Heros. Unfortunately we havent been able to get the dragon out.
Expose thieves I guess is the answer to avoid ops.
with gnomes chain them 1 at a time, they take heavy losses, massacre chains are also very effective against TM gnomes
